The Gary Health Department Vital Records Division will close at 12:00 noon on Wednesday, October 16, 2019 and will remain closed on Thursday and Friday, October 17th and 18th while the city’s Vital Records Staff attends a mandatory State Vital Records Conference in Lafayette, Indiana.
As both Vital Records Registrars will attend the conference, the public is advised that Birth and Death certificates will not be available during the time Health Department staff is away. Regular hours for Vital Records will resume on Monday, October 21, 2019 at 8:30 a.m.
Additionally, the Health Department reminds Gary residents that flu shots are available Free, at the Gary Health Department. Residents may walk-in to get flu shots on Thursdays from 9:00 a.m.-4:00 p.m. Flu shots are also available on Monday, Tuesday, Wednesday & Friday with an appointment.
Residents may call the Health Department for an appointment at 219-882-5565. The Health Department is located at 1145 W. 5th Avenue and closes at 12:00 noon-1:00 p.m. for lunch.
